Do men even have a pelvic floor?
Yes β absolutely. This is one of the most common misconceptions we come across. Every man has a pelvic floor. Itβs a group of muscles that sits at the base of the pelvis, supporting the bladder, bowel, and sexual function. Just like any other muscle group in the body, the pelvic floor can become too weak, too tight, or not function as it should β and just like any other muscle group, it responds well to the right rehabilitation.
How common are male pelvic health problems?
More common than most men realise β and far more common than the number of men who seek help for them. Around 1 in 10 men will experience bladder problems at some point in their lives, and research suggests that 50% of men over the age of 60 will be affected by prostate-related issues. Despite this, many men go years without support, often assuming their symptoms are simply something to be endured. They donβt have to be.
What happens at my first appointment?
Your first appointment at Grosvenor Clinic is primarily about getting to know you and understanding your symptoms thoroughly. Nothing will happen without your full understanding and consent, and you are in control throughout.
Taking Your History
Weβll begin with a detailed conversation covering your symptoms in full β when they started, what makes them better or worse, and how theyβre affecting your daily life. Weβll also ask about your general health, any previous surgeries or investigations, your bladder and bowel habits, your sleep, your activity levels, and any medications youβre currently taking.
This conversation is private, confidential, and completely free of judgement. Speaking openly with us really does help us help you, so weβll always encourage you to share as much as you feel comfortable with.
Physical Assessment
Depending on your symptoms, your physiotherapist will carry out a physical assessment. This is a routine and important part of understanding how your body is moving and functioning as a whole. This may include assessment of your:
- Posture β how you hold yourself at rest and during movement
- Lumbar spine β the mobility and function of your lower back
- Hips β range of movement, strength, and any areas of restriction
- Gluteal muscles β the strength and coordination of your buttock muscles
- Abdomen and pelvis β how your core and pelvic region work together

This whole-body assessment matters because pelvic health rarely exists in isolation β tight hips, a stiff lower back, or weak gluteal muscles can all contribute to pelvic floor dysfunction.
Pelvic Floor Examination
To fully understand how your pelvic floor is functioning, your physiotherapist may recommend an internal examination. We appreciate this can feel daunting, and we will always talk you through exactly what is involved before proceeding. You can ask questions, take your time, and decline any part of the assessment β your comfort and consent come first.
A pelvic floor examination may include one or both of the following:

Superficial genital examination β a visual and external assessment of the perineal region to look for any areas of tension, sensitivity, or changes in tissue that may be relevant to your symptoms.
Digital rectal examination (DRE) β a gentle internal examination carried out by a gloved, lubricated finger to assess the tone, strength, and coordination of the pelvic floor muscles, as well as identifying any areas of tenderness or tightness. This examination is the most accurate way for us to understand how your pelvic floor is functioning and to determine the right treatment plan for you.

Not everyone will need an internal examination, and your physiotherapist will always discuss whether it is appropriate and necessary for your individual situation before anything is agreed.
Will I need an internal examination at every appointment?
No. Internal assessment is typically used in the early stages to get a clear picture of how your pelvic floor is working. Subsequent appointments focus much more on your exercise programme, manual therapy, and monitoring your progress.
Do I need a GP referral to be seen?

How many appointments will I need?
This varies from person to person depending on your symptoms and goals. Many men notice meaningful improvement within a handful of sessions, while others benefit from a longer course of treatment. After your initial assessment, your physiotherapist will give you a clearer idea of what to expect and how often theyβd recommend being seen.
